Hello and welcome to our practice. In order to expedite the appointment setting process we ask that you do the following:
1. Contact the referring physician and have them fax Dr. Lacey a referral along with the last two office notes and any recent test results.
2. Please complete our new patient appointment request form below.

Since many neurological issues can be treated using virtual appointments, we offer telemedicine appointments.
